I want to use my UltraSync Blue (hardware timecode) with the app. Do I have to buy the one-time Pro version AND pay the monthly subscription, or is one of them enough?

You only pay for one of them — not both. MovieSlate's Pro Features (which are required for hardware timecode syncing) are available either as a monthly subscription or as a one-time purchase. You get access to the same features either way; it's simply your preference.
You can even start with the monthly subscription and later cancel it and make the one-time purchase instead.
Note that the MovieSlate app itself is a separate, modest one-time purchase from the App Store, and Pro Features are added on top of that. So for a typical hardware-timecode setup you'd have: the app, Pro Features (subscription or one-time), and your timecode device (e.g. UltraSync Blue). Current App Store pricing is shown on the MovieSlate Store page inside the app and on the App Store listing.
