My scene has both nudity and VFX, but the breakdown column only seems to accept one entry. How do I add more than one tag or prefix to the same scene?

You can enter multiple tags in the field. Tap the ABC button and type in the prefixes you need for the scene. This lets you record more than one breakdown element (for example, both nudity and VFX) instead of being limited to a single entry.
